What's the farthest distance a receiver can be from the dish and the signal still be good? At what distance into a cable run does signal degrade beyond usefulness?
Any recommendations on a very long cable run from dish to receiver?

It depends on where you are located and your budget. You can run it 2000 plus feet if you have the right equipment. Check here http://www.askacom.com and it will give you some good ideas on how to do this. It will give you sizes of amps , ect to fit your application.

i've ran 150 to 200 feet of cable without any amplification.
